About Michael Osborne Case Manager

Michael provides on-going case management support to low-income elders and caregivers residing in Fulton County. He also works on community outreach, using partnerships to enroll low-income, Fulton County residing elders and caregivers in case management services; conducting client assessments and developing care plans based on assessment results; working with clients to implement care plans and follow up with client as required; and conduct in office, in home visits with elders and caregivers, as needed.

Michael's Bio

Michael Osborne, a 1995 graduate of Georgia State University with a Bachelor’s Degree in Social Work, began working at JF&CS as a Case Manager and Information & Referral Specialist for seniors residing in Fulton County.

During his undergraduate studies, he interned in the Older Adult Services Unit of Catholic Social Services. During this time, he was responsible for conducting a falls assessment study to determine if elders identified at risk for falls followed up on referrals to their primary care physician for further medical assistance.

He also volunteers with Grady Memorial Hospital’s Infectious Disease Program, Women and Children’s Unit. Michael completed his undergraduate internship in the Inpatient Infectious Disease Unit at Grady. In 1995, Michael started his tenure with the Department of Family and Children Services (DFCS) determining Medicaid, Food Stamp and TANF benefits eligibility. In 1998, he began working within the Aged, Blind and Disabled Medicaid Programs division at DFCS until 2004. Michael then began working as a Care Coordinator for the Community Care Services Program at Visiting Nurse Health System.

In May 2017, he joined JF&CS to provide on-going case management support to low-income elders and caregivers residing in Fulton County. He also works on community outreach using partnerships to enroll low-income, Fulton County residing elders and caregivers in case management services; conducting client assessments and developing care plans based on assessment results; working with clients to implement care plans and follow up with client as required; and conduct in office, in home visits with elders and caregivers, as needed.
