Care Management

Care Management provides a partner in caring for your elder. The service is a symbiotic relationship between the Care Manager, the client, and his or her family that improves quality of life for all parties involved. After an initial in-home assessment, we perform a complete evaluation and provide you with a personalized Care Plan.

Others are simultaneously juggling career, family needs, and caregiving. Whatever your situation, it can be a struggle for families to provide care for their loved ones. Care Management takes on many of those duties, creating space and freedom for families to focus on making memories and enjoying one another.

One word: CONNECTION. We are connected to our community of resources, and to one another. Aviv is comprised of Licensed Clinical Social Workers, master-level social workers and counselors, and RNs and Clinicians who all work together to create the best solutions for our clients. We are adaptive and nimble to the needs of our clients, and when you work with a Care Manager through Aviv, you're also working with our entire team.

Assistance

From day-to-day tasks like coordinating transportation and doctors' appointments, to making big decisions like new living arrangements, we are here to help.

We communicate important information to family members, and can help mediate difficult conversations.

Advocacy

We ensure that your loved one is receiving the best care possible by being an extra set of eyes & ears at important appointments and during services.

We are involved in your elder's everyday life, and trained to notice changes and make adjustments to the Care Plan when needed.

Education

We help you understand the language and programs involved in eldercare, empowering you to make well-informed decisions.
