JF&CS Transportation Hub

Transportation is a critical part of living independently. Without it, members of our community are socially isolated from friends and family and face barriers to going to medical appointments and accessing important community resources.

Wheelchair-Accessible Transportation Services

JF&CS offers private wheelchair-accessible transportation services. This service is ideal for medical appointments, grocery shopping, social outings, and more. Every ride is provided by trained, vetted drivers, ensuring a comfortable, dignified, and dependable experience.

  • Service Hours: Trips are available Monday–Friday, beginning as early as 8:00 a.m. The latest available start time is 12:30 p.m.
  • Service Area: Within a 20-mile radius of JF&CS (4549 Chamblee Dunwoody Road).
  • Pricing: $100 for one-way trips; $175 for round trips.
  • Extended Distance Fee: Trips exceeding 40 miles (round trip) will incur an additional 15% charge to help cover fuel and driver compensation.
  • Wait Time Fees:
    • If a driver must wait more than 30 minutes before completing a round trip, a $10 fee per 30 minutes will apply.
    • For wait times exceeding an hour, a $15 fee per full hour will be charged thereafter.
  • Payment Options: We accept private pay and Medicaid. Short-term, limited subsidies are also available for eligible clients.

JF&CS also partners with additional transportation providers for trips needed outside our core hours or service area, ensuring continuity of care and access.

For general transportation assistance, please call 866.AGE.WELL (866.243.9355) to explore available options and access registration forms. Rides should be scheduled at least a week in advance to ensure timely service. For Lyft and Uber rides facilitated through On The Go, clients can request a Registration Form by calling the same number. Once the form is completed, clients will be contacted by Ernie Ball, Transportation Manager, to set up billing and prepayment arrangements.

Subsidies may be available for low-income clients upon verification.
