About Samantha Freeman, MS, CPG, CDP Case Manager
Samantha Freeman, MS, CPG, CDP, is a Credentialed Professional Gerontologist and a Certified Dementia Practitioner. She is a case manager and the memory support services coordinator.
Bio
Samantha is passionate about partnering with individuals living with dementia and their support systems to create meaning and enjoyment in their lives throughout the aging and dementia journey. She brings over 10 years of experience in the field of aging, earned a bachelor’s degree in psychology from the University of Georgia, and a Master of Science in Applied Gerontology from Brenau University.
She is a Credentialed Professional Gerontologist and a Certified Dementia Practitioner. Samantha uses her experiences and skills along with current research evidence to inform and guide her approach to working with older adults. She entered the aging field as a Certified Nursing Assistant during college. Using the insight she gained as a CNA, she is able to easily and effectively develop relationships with family and paid caregivers. Working as a Life Enrichment Leader in independent and assisted living communities has focused her attention and passion on finding non-drug interventions for managing symptoms and reactions associated with dementia progression that can be shared and implemented by caregivers.
At JF&CS, Samantha also coordinates the Memory Support Services programs, designs and delivers in-home Memory Support Coaching to caregivers, and provides case management services to older adults. Her goal is to help change the perception of aging and dementia in our community.
