When Jeff was growing up, his parents were his world and his primary source of care. For decades, they supported him as he navigated life as an adult with Intellectual & Developmental Disabilities (I/DD). But when Jeff reached his 50s, his family faced a profound shift: both of his parents developed dementia and could no longer care for him. That’s when Sarah South, Aging Care Services Manager, for Aviv Older Adult Services stepped in.
Understanding the importance of keeping the family together during a difficult time, Sarah coordinated housing for Jeff and his parents in an eldercare facility where each of them could receive the support they needed. It was a solution that prioritized both care and connection, ensuring that even as their needs changed, they could remain close.
In the time that followed, Jeff remained by his parents’ side as their conditions progressed. When they passed away, Jeff was navigating life without the people who had always anchored and cared for him. The grief, combined with the sudden change in his daily life, led to increasing isolation, and his physical and mental health began to decline.
Recognizing the urgency of the moment, Sarah connected Jeff with one of our I/DD Case Managers, who worked with him to build a long-term, coordinated plan for stability and support. Today, Jeff’s life looks very different.
Each weekday, he attends our Community Access Group (CAG), where he spends time with peers in a welcoming, structured environment. He lives in a residential host home through the Zimmerman-Horowitz Independent Living Program, sharing a suburban home with roommates and receiving the support he needs to thrive. With the help of one of our dedicated transportation professionals, Jeff travels safely between home and the CAG, giving him both independence and consistency.
Jeff’s days are now filled with movement, social engagement, and meaningful routine. He participates in community outings, builds friendships, and experiences the kind of belonging that everyone deserves. His story is a powerful reminder that as needs change, the right support can make all the difference to create a life but filled with community, dignity, and connection.
